Project Summary

Northwest Healthcare and CHS selected NexCore to develop an on-campus Post Acute Center of Excellence providing multiple points of post-acute care under their control to reduce patient readmissions and achieve better patient outcomes. Located on the Northwest Medical Center campus, adjacent to the hospital and the Gateway Medical Plaza, the two-story facility will house 30 beds dedicated to transitional care and 30 beds allocated towards inpatient rehab. Though operations will share core services, the facility will hold two licenses, one for the hospital and one for skilled nursing.

Northwest Transitions broke ground on February 11, 2020, and was completed in 2021.
